5 Warning Signs Your Leg Pain Is Caused by Blocked Arteries (Not Just Tiredness)

Many patients suffer PAD for years, dismissing the symptoms as "getting older." These five signs distinguish arterial leg pain from ordinary fatigue — and should prompt immediate vascular evaluation.

Sign 1: Pain That Always Starts After the Same Distance

PAD pain (intermittent claudication) is remarkably consistent: it always begins after walking roughly the same distance — say, 200 metres. This predictability is because the narrowed artery can always supply enough blood for rest, but always fails at the same workload. Ordinary tiredness is unpredictable and varies with how rested you are.

Sign 2: The Pain Completely Disappears Within 5–10 Minutes of Rest

Stop walking, rest for a few minutes, and the pain goes entirely — then returns after the same distance. This "cramping-rest-relief" cycle is the textbook pattern of claudication. Muscle strain from exercise does not completely resolve in 5 minutes.

Sign 3: One Foot Is Always Colder Than the Other

Temperature asymmetry between feet is a reliable sign of reduced arterial flow on the colder side. Feel both feet with the back of your hand — in PAD, the difference can be dramatic.

Sign 4: Leg Pain That Wakes You at Night

Rest pain — burning or aching in the foot and toes at night — is a sign of advanced PAD (critical limb ischaemia). Hanging the foot off the bed provides slight relief because gravity slightly increases blood flow. This is an urgent sign requiring prompt vascular evaluation.

Sign 5: A Small Cut That Takes Weeks to Heal

A minor cut, blister, or scratch on the foot that is slow to heal — especially in a diabetic patient — is a warning sign of critically inadequate blood supply. Normal wounds heal within 7–10 days.
